Caregiver Shift Handoff Template
A structured handoff note that prevents the information reset between visits

Fill this in at the start of every visit so the handoff is timestamped and attributable.

Check off routine activities you completed and add any notes about what was done, how it went, or anything worth passing along.
3 What changed or needs attention
Note anything that was different from your last visit or from the client's normal baseline. This section helps the next caregiver (or the family) see the trajectory over time.
