Houston, TX Care Coordination Resources

A locally specific starting point for coordinating a parent or partner’s care in Houston — major health systems, caregiver support groups, aging resources, meals and in-home help.

Non-clinical administrative coordination. Not an emergency service.

Starting point: This page is a curated set of common resources—not a complete directory, ranking, or partner list. Please confirm details with each organization. Suggest an update.

Local snapshot

Houston’s care landscape is anchored by the Texas Medical Center — a dense cluster of major hospitals where appointments, parking, and building-to-building transfers can take real coordination. Families often bounce between Houston Methodist, Memorial Hermann and Baylor St. Luke’s on the same campus, while Harris Health (Ben Taub) is a key public safety‑net option nearby. Outside the core, clinics and rehab sites spread fast toward the Loop, Katy, The Woodlands and Clear Lake, which can add drive-time and “which location is this?” friction for out‑of‑town helpers.

For aging support, the Houston‑Galveston Area Agency on Aging (H‑GAC) is a practical first call for benefits counseling and local programs, and meals/food programs are run through well‑established local partners. If you’re coordinating from another state, plan on collecting portal access, release forms and a single shared “open loops” list so siblings and local caregivers can stay aligned.

Where to start (especially if you’re remote)

  • Start a single shared “open loops” list (tests to schedule, forms to sign, meds to refill, follow‑ups due) and share it with siblings/local helpers.
  • Ask for portal access early: request proxy access (or invite access) so an out‑of‑state family member can message the care team and see visit notes.
  • Keep a simple “med list + allergies + pharmacy + primary care + specialists” one‑pager. Update it after every visit or discharge.
  • For hospital stays, request the discharge summary, current med list, and follow‑up instructions before leaving. Get the name of the unit/social worker who handled discharge planning.
  • When you need unit or nurse‑station contact details, call the hospital’s main operator and ask to be transferred (numbers change frequently).
  • Use a release‑of‑information form (or equivalent) for each system so staff can speak with the right family members — especially when siblings are sharing duties.
  • Document basics in one place: insurance member IDs, Medicare number, advance directive status, and a “who can make decisions” note (healthcare proxy/POA).
