What you get. A named Care Continuity Partner, assigned from Expanded and above, and the Averyn app where the two of you work from the same thread and calendar. Someone sent you this? If an adult child or a friend sent you the link, nothing here starts without you: you set the priorities and give permission for new actions, and the authorization is signed by you, or by someone holding a power of attorney, guardianship or conservatorship for you. The people you name are activated at your direction, and your Care Continuity Partner supports whoever steps in rather than taking over from you. Nobody comes to the house.

You decide what happens, and who steps in.
Before your Care Continuity Partner starts anything new, we confirm it with you, so nothing happens without your say. You get written summaries of what was requested, what came back and what is still open. If you add a spouse, a friend or an adult child to the account, they read the same updates you do, and you can pause or revoke that access whenever you want.
You also name the person who would step in if you could not run this yourself for a while, and what they get is a head start rather than a pile of paper. They receive the Averyn Ready packet, which puts conditions, medications, allergies, surgeries and the people to call in plain English, and a Care Continuity Partner who already knows the situation. It is built from your Record Vault, the source-labeled set of records we request with your authorization from the providers and portals you name. You decide who sees it, and when.
The plan is not a document you file and forget: your Care Continuity Partner keeps it current as things change. If an event comes, Averyn can activate the people you named in advance, a daughter, a hired care manager, a financial advisor, at your direction and without you handing over control, and then acts as a resource to whoever steps in.
