Other support roles advise you on what the system offers. Averyn executes on what your family needs — calls, portals, paperwork, follow-ups, vendor coordination — so you can focus on being present instead of being on hold.
Onboarding slots are limited each month — we take on new households at a pace our team can support well.
How it works

Averyn Continuity Plans
After your Record Vault is complete, choose the level of administrative execution that matches your situation. Your Care Continuity Partner works on your priorities — not a system's — and every plan keeps your Vault current.
What drives Continuity Plan level
- More providers and more portals = more coordination and loose ends
- More family members involved = more alignment and communication work
- Transitions (hospital, rehab, new diagnosis) = more time-sensitive tasks
- In-home helpers or rotating caregivers = handoffs, exceptions, and daily management needs
Plan level increases when the household has more loose ends and more people to keep aligned.
Maintenance mode — keep things current.
- Full Averyn.app access — dashboard, calendar, messaging, Record Vault, and more
- Vault maintained and accessible
- Care team directory kept current
- Scheduled check-ins and written updates
- Light human support with escalation
Best fit: Light or stable needs — often added alongside Expanded or Dedicated for a second supported person requiring care in the same household.
Essentials is typically offered by recommendation after we understand your situation; it's also commonly used as an add-on for an additional Supported Person.
Keeps small things from becoming big problems
Records stay current, the care directory stays accurate, and open items stay visible — even when life is stable.
Several providers, mostly stable, but steady coordination between visits.
Everything in Essentials, plus:
- Dedicated Care Continuity Partner assignment
- Full Care Continuity Partner-led record requests and tracking
- Ongoing follow-through on loose ends
- Weekly written household updates
- Care summaries after key appointments and events
- Multi-stakeholder coordination
- Family Alignment Call — 1 quarterly at request
Planned rhythm: An every-two-week working conversation with your Care Continuity Partner, plus coordination touchpoints as needed.
Scheduling, transportation, forms, refills, portal questions, and family updates — your Care Continuity Partner works the open loops so they don't pile up.
Referrals, follow-ups, and family updates that actually happen
Your Care Continuity Partner persistently tracks every open item. Your family gets one clear written update instead of fielding separate "what did the doctor say?" calls — and the app keeps everyone aligned without you relaying it.
Multiple active providers and systems — the situation changes often.
Everything in Expanded, plus:
- Priority Care Continuity Partner assignment and response
- Weekly working conversation with your Care Continuity Partner
- More active coordination and support between calls
- Family Alignment Call — 1 monthly at request
- Most add-ons bundled (see grid below)
Planned rhythm: A weekly working conversation with your Care Continuity Partner, plus more active support between calls.
Specialist visits, therapy, transportation planning, caregiver coordination, recurring follow-up — new loose ends appear regularly and need weekly active management.
Persistent, high-frequency coordination you couldn't sustain alone
Every open item is tracked with deadlines and follow-up cycles. Siblings and helpers see the same updates through the app — no group texts, no repeated calls, no half-informed family threads. The work doesn't just shift; it gets done more consistently than it could without a system behind it.
Savings shown vs month-to-month pricing. You don't have to pick a plan before you reach out — we recommend one after we understand your situation.

From executive assistant to assistant manager
Essentials, Expanded, and Dedicated act like an executive assistant for your care logistics — handling calls, portals, follow-ups, and updates on your behalf. Anchor shifts to an assistant manager role: running daily caregiver coordination, maintaining the Care Ledger, and keeping the home-based plan executable — not just handling tasks, but running the system.
Home-plan management, caregiver handoffs, and daily exceptions.
- Daily check-ins with caregivers, home health staff, or family caregivers
- Care Ledger — maintained daily so new helpers don't restart from zero
- Appointment attendance by phone/video, capturing instructions in real time
- Dedicated callback line + voicemail — shields the Supported Person from the phone/portal avalanche
- Facility & transition coordination with daily updates during business hours
- Caregiver handoff discipline — onboarding new helpers to the Ledger on day one
Anchor is for households when daily coordination is required.
- Staying at home depends on daily coordination — rotating caregivers, active transitions, or a plan that drifts without daily attention
- Caregiver turnover is frequent enough that mistakes and rework happen
- The Primary Contact is spending significant daily time just getting a reliable status update
- Active transition window: hospital → home, SNF/rehab → home, new meds, new provider network
Most families use Anchor during fragile seasons, then step down to Dedicated once things stabilize.
Non-clinical · No monitoring · No travel · Remote-first
Averyn provides administrative coordination. We do not provide medical advice, clinical triage, emergency services, or 24/7 monitoring. If you believe there is an urgent medical concern, call 911.
It's not just about hours. It's about what actually gets done — and what quietly stalls when no one is tracking it. Referrals that expire. Authorizations that lapse. Follow-ups that should have happened last week. Without a persistent system, these things don't just take your time; they don't happen at all.
Your Care Continuity Partner works from a purpose-built platform that tracks every open item with deadlines and follow-up cycles. That means work gets done with a consistency and cadence that's difficult to replicate with phone reminders and sticky notes — no matter how organized you are.
And there's time you're saving that you might not see coming. When your Care Continuity Partner sends a structured weekly update through the Averyn app, every sibling and helper sees the same information at the same time. That's the end of the "what did the doctor say?" phone chain, the half-informative family group text, and the shared calendar that nobody updates. The coordination load on the wider family drops — not just on you.
Other support — insurance case managers, CCM programs — advises you within their system's priorities. Averyn executes on yours, across every system, provider, and portal — and gives your whole family a shared view of the work.
Imagine next week: the referral is tracked, the portal message is answered, the follow-up is confirmed, and your brother already knows what happened at Tuesday's appointment — because it was in the app before he could call to ask. That's what this buys.
Caregiving admin averages 27 hrs/week (AARP/NAC 2025). See the breakdown →

Commitment and cancellation
Annual commitment
12-month term, billed monthly at the lower annual rate. If you cancel before the term completes, a one-time early-exit adjustment applies:
| Plan | Early-exit adjustment |
|---|---|
| Essentials | $89 |
| Expanded | $374 |
| Dedicated | $1,199 |
This adjustment is waived in cases of the Supported Person's passing or a qualifying change in care level. Contact us and we'll handle it with zero friction.
Month-to-month
No commitment required. 30% premium over the annual rate. Cancel anytime — no early-exit adjustment.
Why fixed monthly pricing instead of hourly
Many care-adjacent services — geriatric care managers, patient advocates, elder law consultants — charge by the hour. Averyn uses fixed monthly pricing by design.
The reason is simple: hourly pricing creates a perverse incentive. When every portal message, callback, and follow-up is a billable event, families start rationing contact. They hesitate to ask. They try to handle the "small" things themselves. And the small things are exactly what pile up into the administrative burden that brought them to Averyn in the first place.
Fixed pricing means your Care Continuity Partner is working the list — not waiting for you to decide whether today's question is worth a billable hour. The relationship feels like ongoing support, not a meter running.
Family access is included
Family access is included for everyone involved in care or who should stay in the loop, at no additional cost.
This can include family members, private-duty caregivers, and other trusted helpers involved in day-to-day support. Everyone can see requests and updates; helpers can message your Care Continuity Partner, while you remain the primary point of contact.
Access is through the Averyn App — a secure, browser-based platform that works on any phone, tablet, or computer. No app store download required. Each person gets their own login.
Many families share the cost among siblings or other family members. Averyn bills to one card — your family handles the split however works best. The Primary Contact remains the decision-maker on the account regardless of who pays.
This is where it gets simpler
You've been carrying this alone — between meetings, after hours, on weekends. It doesn't have to stay that way.
Most families start with a quick intro call. Already know you're ready? The $199 deposit skips the call and locks in your start date — it applies to the $999 Vault fee. Limited spots each month.
