For anyone pricing this out for themselves, a parent, or someone they love.

Fixed monthly pricing. No hourly billing.

Every household starts with the Record Vault at $999 to build; then $59 to $799 a month on an annual commitment, or $2,999 a month for Anchor, three-month minimum. Private pay, not insurance.

Administrative coordination and follow-through. Not medical advice, not clinical monitoring, not emergency response.

Step one

Every household starts with the Record Vault.

One-time build

Averyn Record Vault

$999 one time

The build produces eight pieces: Care Team Directory, Permissions and Access Map, Authorization Packet and Tracker, Records Request Tracker, Baseline Records Repository organized by provider and date, the plain-English Averyn Ready packet, Medication List, and one Family Alignment Call. One refresh is included when late records arrive. It is yours to keep whether or not you continue.

  • The Vault is the first invoice; a monthly fee starts only with a Continuity Plan.
  • $199 reserves a start date and applies to the $999.
  • High Complexity Capacity, plus $400, when any of these apply: five or more active providers (primary care plus the specialists actively consulting) in the last six months, three or more portal systems, a hospital stay, rehab, surgery, a move or a new diagnosis inside 60 days either way, or two or more insurance contexts that materially affect record flow. Care Complexity Assessment.
  • Additional Supported Person in the same household, plus $400 each.

Ten business days from kickoff is the target for the initial delivery. Meet the participation requirements and, if we miss the agreed date, you choose a full refund or a full credit good for 12 months. Read the guarantee.

Reserve a start date, $199 See the first two weeks

With your authorization your Care Continuity Partner requests records from the providers and portals you name. The Supported Person is whoever the records are about, and that can be you; a household of one is a household.

An employer can fund part of a household's start through Averyn Keystone.

Step two

Then a fixed monthly fee, or nothing at all.

A Continuity Plan is optional. You need not choose first.

A household of one

Running your own care rather than someone else's? Expanded is the usual fit, and month to month is there from day one.

The first year

A common first year is the Vault at $999 plus twelve months of Expanded on the annual commitment: $3,987. With the High Complexity Capacity add-on, $4,387.

Family access and who pays

Everyone you name gets their own login to the Averyn app at no extra cost, as patient, primary caregiver or caregiver. Many families split the cost among siblings: Averyn bills one card and your family handles the split however works best. The Primary Contact stays the decision-maker on the account whoever pays.

What decides the level

Not how often you want to talk, but how much coordination the household takes: providers and portals, family members involved, transitions under way, helpers on a rotation. The Care Complexity Assessment is seven questions and takes about two minutes.

Beyond the three plans

Anchor is a different operating model.

Not more of Dedicated. It is for a household where the home plan needs a daily rhythm to stay coherent: rotating caregivers, home health in the house, or an active transition after a discharge.

Add-ons

What costs extra.

Where a monthly inclusion applies, extra usage is billed at the published rate. Inclusions reset monthly and do not roll over. Appointment Support, Family Alignment Calls and hospitalization assistance are priced in the table above.

Add-on Price Essentials Expanded Dedicated Anchor

Transportation and other vendors are chosen by the family and are responsible for their own performance.

A second person in the same household

The discount is for one household: one Care Continuity Partner, one calendar, one bill. Monthly add-ons run $30 to $599 on an annual commitment.

Terms

What you are committing to.

Commitment and leaving early

The default is a 12-month commitment billed monthly; month to month costs 30 percent more, with no commitment and no exit fee. Leaving an annual commitment early costs 150 percent of one month: $89 on Essentials, $374 on Expanded, $1,199 on Dedicated, plus any Supported Person add-ons. That fee is waived if the Supported Person dies, or when a change in their care setting leads the household to downgrade or cancel.

Prepay 12 months

Prepay a year at the annual-commitment rate and you receive two extra months, 14 in total: $708 on Essentials, $2,988 on Expanded, $9,588 on Dedicated. Bonus months come after the 12 paid months, have no cash value, and are forfeited if service ends early.

Anchor

A three-month minimum term, then month to month with 30 days' notice.

Keeping the Averyn Concierge Line

If you would rather own the number and the inbox, these options cover the contact path only; they do not extend Care Continuity Partner service.

Option Price What it is
Portable Concierge setup $100 one time A family-owned Gmail address and Google Voice number configured with the same routing, so the contact path is yours to keep.
Port-away assist $50 one time Help transferring the Averyn-issued number to a family-owned account if a plan ends.
Proxy email forwarding Included for 12 months after a plan ends Inbound proxy email forwarded to an address you name for one year, with a one-time inbox export.
Phone forwarding after a plan ends $10 a month Keep forwarding inbound calls on the Averyn-issued number, month to month.

Why it is priced this way

Fixed monthly pricing instead of hourly.

Geriatric care managers and patient advocates usually charge by the hour, and families start rationing contact. A fixed fee means your Care Continuity Partner is working the list, not waiting to see whether today's question is worth a billable hour.

Caregiving averages 27 hours a week, on AARP and the National Alliance for Caregiving's 2025 count.

Price it against your actual week.

Fifteen minutes on the phone, and we will tell you which plan we would suggest and what it would cost. The FAQ answers most questions.
