The facts

What it costs. Private pay, no insurance billing. The Record Vault, one organized set of your records with its source labeled, is $999 to build. Ongoing Continuity Plans are a fixed monthly fee, $59 to $799 a month on an annual commitment, depending on how much is happening. See plans and pricing.

What the coordination looks like.

  • Pediatric to adult

    A young man in his early twenties with a developmental disability, moving from pediatric to adult care. His surgery can only be done at one hospital, his parents work on opposite coasts, and the weekend caregiver just gave notice.

  • One diagnosis, three portals

    A parent of two children with the same rare condition, spread across three portals and two hospital systems. Every form gets filled out twice, and the history re-explained at every referral.

  • A diagnosis in his fifties

    A husband with a new neurological diagnosis and a job he intends to keep. His wife books the referrals between her own meetings, and the paperwork arrives faster than they can file it.

What is the same

The same operating model as every other household.

You start where every household starts: the Record Vault: records requested with your authorization from the providers and portals you name, organized and labeled with their source. The authorization is signed by the person you support, or by someone holding a power of attorney, guardianship, conservatorship or parental rights for them, and it names the providers Averyn may contact.

Then your Care Continuity Partner runs the follow-through week to week. The Averyn Concierge Line gives providers, pharmacies and vendors a number to call that is not yours.

Allergies, conditions, medications, the pharmacy, the people to call and who the authorized representative is. Each fact carries its source; your family corrects and adds what the records miss.

What is different

Four things this household needs that most do not.

  • Caregiver roster

    At your direction we confirm scheduled coverage and contact the backups you name when a shift opens. Staffing the roster stays yours, and Averyn does not employ, supervise or place caregivers.

  • School health and therapy paperwork

    With your authorization we relay forms, orders and scheduling between the school health office, the therapy clinic and the prescribing office, and track what is outstanding.

  • State programs and waivers

    At your direction, we organize and track the paperwork, renewal dates and document requests for the programs you are in or applying to. We do not advise on eligibility or represent you in an appeal.

  • Proxy access

    With your authorization, we set up portal proxy access during the Vault build, and where a portal does not support proxy access we note the workaround and who holds the credentials. When a child turns eighteen, portals and offices need the adult's own authorization or an authorized representative's; we organize whichever authorization your family and the adult have chosen and set it up with the rest. We do not advise on guardianship or supported decision-making.


What we do not do

Where we stop.

IEPs and 504 plans are out of scope: no IEP meetings, no IEP advocacy. We deliver school documents, confirm school-held medications, handle task work you ask for, and schedule around the school day.

Clinical decisions stay with you and your clinicians. We relay what you ask, as reported.

We work business hours, not around the clock.
